Referral to the clinic must be made by an outpatient nephrologist and must meet these criteria:
We will provide CKD management to patients who are receiving immunosuppressive therapy for glomerulonephritis or transplant. Patients will continue to receive follow-up care from their referring MD for management of immunosuppressive therapy.
We will accept new patients only when the above information above has been provided.

Please note: All patients with an eGFR of < 12 who are symptomatic are considered urgent and would not benefit from this clinic. Such patients should be referred to the ER. Asymptomatic patients will be discussed on a case-by-case basis, but acceptance is not guaranteed unless eGFR is > 15.
