​​​​​​Physicians who wish to refer patients for consideration of radiosurgery are requested to fax clinical information as well as imaging to the numbers listed below.  Cases are reviewed every week and patients are seen promptly. 

Referrals from all physicians are accepted.

The Gamma Knife Centre accepts referrals for Ontario residents and out-of-province Canadian residents. Referrals for non-residents or uninsured patients are assessed on an individual basis.

Patients may be referred to the Gamma Knife Centre for consideration of radiosurgery for the following clinical indications:

  • Patients for whom medications or surgery don't work for their trigeminal neuralgia
  • Patients with brain or dural vascular malformations (AVM) suitable for radiosurgery
  • Patients with specific types of brain tumours may considered for treatment with Gamma Knife radiosurgery
  • Movement disorders (tremor) and specific types of severe pain
  • Patients with specific types of epilepsy

 

Urgent Referral ​

If your patient needs immediate, urgent care, please do the following:

  • Please fax a referral letter to 416 603 5155 and​ label urgent.​


Follow-up with a call to the Gamma Knife Centre at 416 603 5445 to let us know that you have faxed in a urgent referral.

 

Standard Referral ​​​​

To complete the standard referral process, please fax a referral letter to 416 604 5155.

The form will be reviewed to ensure​ that it is complete; if incomplete, the form will be returned to you. 

Cases are reviewed every week and patients are seen promptly. Our office will call you with information about your patient’s first appointment. ​

Your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring. 

It is extremely important to ensure that your patient brings the following, as these will be required at their first appointment. 

  • List of medication(s)
  • Recent imaging tests

 

Checklist for a Complete ​​​Referral 

In addition to the referral letter, make sure to include: ​

  • Clinic notes 
  • Relevant imaging​​​
