Urgent Referral

If your patient has thunderclap headache please send to the emergency room to rule out conditions like subarachnoid hemorrhage.

For urgent cases, please contact Dr. Jennifer Robblee directly by confidential email to email the nature of the urgency, and based on the information given the referral will be triaged as appropriate.

Standard Referral

To complete the standard referral process, first download and complete the KNC Headache referral form. Ensure the form is complete including name of the family doctor if this is not you. Please also include the patient's first language and whether they are fully fluent in English so the clinic knows whether to book a translator. Please give your patient 3 copies of the pre-consult headache diary and instruct them to complete this diary. They are to bring it to their first appointment.

Once you have completed the form, fax it to the Headache Clinic  at KNC at 416-603-5768 or email it directly to Dr. Jennifer Robblee.

SCOPE family doctors can contact Shawna or Maggie for further information before referring if necessary.

The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.

It is extremely important to ensure that your patient brings the following items as these will be required at their first appointment.

Checklist for a Complete Referral

In addition to the completed referral form, make sure to include:

  • All previous clinics notes from neurology, pain clinics, concussion clinics, ENT, dentistry, neurosurgery related to headaches or facial pain
  • Reports of MRI or CTs
  • List of the patient's medications including previous medications tried for headache
  • List of the patient's past medication history

If this information is not included with the referral form, your referral may be returned as incomplete.​
