Transplant Clinic

The Ambulatory Clinics of the Multi-OrganTransplant Program are designed to meet the follow-up health needs of organ transplant recipients. All patients transplanted at UHN have a designated Transplant Coordinator within the Ambulatory program who assists in managing their care along with the multidisciplinary health team. Each organ group has a specific time and weekday for clinic, with appointments scheduled through their coordinator.

Easy Call

Easy Call is an efficient communication system for transplant recipients and their transplant coordinators. The system allows patients to leave messages for and or pick up messages from their transplant coordinator. Transplant staff check Easy Call once per hour from 8 am to 4 pm Monday – Friday.

Easy Call can be used to:

  • notify your transplant coordinator of any non-urgent changes to your health
  • ask questions about your medications or health status
  • ask for prescription refills
  • pick up messages from your coordinator
Each patient is given their own personal identification number or PIN upon discharge from the inpatient transplant unit. This allows them to sign into their Easy Call mailbox and retrieve or place messages. Patients are responsible for checking their mailbox at least twice a week. Daily mailbox checks are advised for patients transplanted within the past 3 months or those with recent health issues.
