The Ambulatory Clinics of the Multi-OrganTransplant Program are designed to meet the follow-up health needs of organ transplant recipients. All patients transplanted at UHN have a designated Transplant Coordinator within the Ambulatory program who assists in managing their care along with the multidisciplinary health team. Each organ group has a specific time and weekday for clinic, with appointments scheduled through their coordinator.

| Easy Call Easy Call is an efficient communication system for transplant recipients and their transplant coordinators. The system allows patients to leave messages for and or pick up messages from their transplant coordinator. Transplant staff check Easy Call once per hour from 8 am to 4 pm Monday – Friday.
