Patient and Care Giver Information - Referral Process

Referral Process

Your doctor can refer you to one of our specialists by sending a referral letter and other information like:

Your personal information (e.g. name, address, age, telephone number, marital status, etc.);
Your health insurance card like OHIP.
Your health information like a list of any other sickness and operation you've had before;
Your pathology report. This report tells us what kind of cancer you have;
Diagnostic Imaging Films given to you to bring to appointment like a CT scan or X-ray

It may take us up to two weeks to see you when we get your referral letter.
