We work in the community with people who have severe and persistent mental illness and therefore require a high degree of personalized help and contact with community health professionals who can provide one-on-one attention when needed.

Provide treatment, rehabilitation therapy and support.
Work with the client to develop a customized, individualized plan for living in the community.
Help develop vocational, interpersonal and social relationships by encouraging participation in community activities.
Monitor, adjust and deliver medication to the client.
Help the client develop living skills.
Advocate on behalf of the client
